Patient Instructions

• Step 1: Register using the QR code or invitation from your healthcare provider, or Contact Us for assistance.
• Step 2: After registering, your parent will receive a text message with a parental consent form that needs to be signed.
• Step 3: Login to the platform with the credentials you created when registered.
• Step 4: Select a provider for your session from the directory inside the platform and schedule anappointment.
• Step 5: Answer the surveys the provider sent beforehand the appointment.
• Step 6: At the appointment time, click the link that comes with your text reminder or login and clickthe Start Video button. Contact your doctor's office to review the report.
